Scripter required for email form - quote needed

Hi guys,

I don't have the time nor inclination to build this web contact form. I just need a simple questionnaire form that emails me the results

It is going to need about 10 fields (or questions with boxes for answers), then when clicked emails me the results, i also might need to update the fields in the future to add more questions - it also needs to forward onto a page afterwards
