09/21 resulted in the new plan rate being charged
from 09/21 - 10/02. The qualifying National
Account Discount is included in the Monthly
Charges section.

7. Data Unlimited for iPhone 4S 8.69CR
This plan is $30.00 per month. Removal of this
plan on 09/22 resulted in a credit for the days
the plan was not active. The credit is offset by
the days not eligible for the National Account
Discount. 09/22 - 10/02

Watch out for the error on your September statement if you upgrade to iPhone5 with the LTE plan, I call the CSR and point out the error and they said they will credit me back $15 for my inconvenient.

Between 9/21 to 9/30, I was charge $12 for the LTE data, but only getting $8.69 credit for the 4g data, keep in mind that both the plan are $30 and there is 30 days in September, I just lay out the fact to the CSR, never get angry or raise my voice, and he apologize and credit my account.
