I’ve worn my watch to bed for years. With Sleep Focus (and Bedtime before that), it was fine - the always on screen would turn off, and notifications were silenced until the watch woke me up at the scheduled time. Last night, however, I received two texts around 2:30AM and the watch vibrated and then displayed both of them! Was there a setting change I missed somewhere? Sleep focus was definitely on, with the watch screen otherwise turned off, and the sleep alarm went off like normal this morning…

WAG, "Share Across Devices" not toggled on? If on, might try flicking off and back on, see if that fixes whatever plist might be broken.

Otherwise, Focus looks correct.

When something doesn't work on Apple devices, I always make sure I've downloaded the latest software. If yes, then shut down and start again.

Sleep focus "should" work.

Good luck.
