I've already upgraded my devices to newest OS as Macbook Pro 16 inch, iPad Pro 11 inch (2nd generation) and iPhone XS Max and meet an issue about icon missing. Someone else meet the same trouble?

My iPad/iPod/iPhone all show their respective icons, but I have no user icon in my system preferences, but I've never selected one for there...if you hover over it you can see the 'Edit' option to add one.

these are unrelated issues (ios & mac os icons). and as tyc0746 suggested, add an image in the icloud settings... even if there was one there previously.

it's no problem before upgrading macos. normally, the avatar that belongs to your apple id will be displayed there by automatically. I tried to add an image but, it also will be lost after mac going to sleep.

do you have an image on your card in Contacts? try putting one there, see if that does it (make sure Contacts is on in icloud's settings)...

Maybe you dont understand what I’m talking about and you cannot fix it.

Have you tried adding your image to online iCloud. That often fixes this issue on your System Pref pane. Alternatively, try adding it to Message on your iPhone where it will ask if you want to "Use everywhere".
