my phone is on ring loud, all button noises on, and not using bluetooth:

when i get a call, i slide the bar to answer, and i can't hear the person at all... unless i put it on speaker. Speaker works and I have to have all phone conversations this way - total PITA.

Any suggestions?

Version? Is it hacked (unlocked, SIM unlock, installer, etc.)? Has it always done this, or did it start after installing a specific app (if you use installer)?

If it started occurring after you installed a specific app, uninstall that app, reboot and see if that is what it was.

If you don't use installer, or don't have a hacked phone, try a restore and see if that does it. If it doesn't, you may have a defective unit, and the Apple Store should swap it.
 
I had a similar problem with my iphone where several times I couldn't hear anything coming out of the headset or the external speaker. I couldn't hear anything during conversations and no music would play either. Never tried the speaker option.

The way I fixed it is that I got my headphones and plugged it in and out of the jack until music started playing on the external speaker. I think it has something to do with the iphone getting mixed up when you're playing music and unplugging your headphones. This happened to me enough times to alarm me and I was thinking of exchanging it through their repair service. Give that a shot.
