Strange app showing up in screen time

The following app (see attached picture) is showing up in the list of apps for app limits in screen time has any one else’s had something similar? Is it something to be worried about such as spyware? This is on an iPhone 6 with ios12 GM.

its either an app that (1) is still on yr phone but for some reason its name and icon were not properly able to be resolved.
or, (2) its a deleted app that has has not been completely deleted.
i have had these sometimes, and both of these cases were nothing to worry about, and eventually got flushed out of the system somehow.
