I jailbroke (untethered) my iPhone 4 (5.01) yesterday. I have installed:

f.lux
iBlacklist
OpenSSH
Winterboard (with one theme)

Just after midnight (yes, just as soon as 12:01am rolled around) four blank(white) icons appeared--AdSheet, FieldTest, iOS Diagnostics, and Setup.

Adsheet turns the screen black briefly then back to normal.
Field test brings up a menu for a variety this related to UMTS, GSM cell environments.
iOS Diagnostics right now has the "send info to Apple" screen. Though it had something else the first time I opened it. But, I was jacking around with it and don't remember what it was.
Setup opens the Wi-Fi Networks menu of Settings.

Any ideas of what these are, where they came from, and what I should (could) do with them?

It happened alot in 5.0.1 on my iPhone 4s when ever you make changes to the springboard, like install or removing apps.
 
I found it to be caused by libhide, when I used the SBSettings option to hide icons. As soon as I stopped using SBSettings to hide icons and switched over to Springtomize 2, I have not had this problem reoccur.
