Had the same problem. T-Mobile rep told me that it was a known issue about the "one time payment option" not discounting the $99. She advised me to select the Installment Plan instead.
Yes. You need to meet this two conditions for unlock:
* iPhone must be paid in full
* You must have been with them for more than 40 days
They are quite strict about this. You can get the $100 deal starting on the installment plan, and then just pay the phone off in a few days. But you'll have to wait 2 billing cycles for them to unlock the phone.
But with the installment plan, if you leave their wireless plan within 24 months there MAYBE a $2000 cancellation fee.
Worst, there maybe a $200 fine, please read the find print before final check out.
I'm assuming you meant $200? And are you referring to an ETF? T-Mobile does not have an ETF fee.
I saw on Twitter that they announced that they were not done discounting phones with just the iPhone.

It all depends if you're well qualified or not. You probably need to put down 50% of the device now. That changes once you increase your tenure with T-Mobile.

I don't understand this whole tied down /phones are locked to one carrier. I thought Apple made all iPhone 6(+)'s universal to be used on any carrier with their universal GSM SIM and built in CDMA chips? Same way the iPad Air 2's are?
The hardware is universal. Carrier locking is the carrier's way to make sure equipment that they are subsidizing (or financing for you at 0% interest) remains on their network at least until it is paid off. That way they can recoup the cost of subsidizing the device by raking in service fees while the device is being used on their network.
Otherwise you could have people using their friends' or family's upgrade on, say, AT&T and then use the device on T-Mobile with cheaper service. In this case AT&T would lose money since they discounted the device with the intention that it would be used on AT&T, but instead someone took it to T-Mobile so they won't be able to make that money back.
I'm not saying I agree to it, but it makes sense from their business point of view. Usually once the device is paid off (or contract term is up) you can request that it be unlocked so you can use it with another carrier.

So here's another question, If one purchased a full priced "Sprint" iPhone 6 at the Apple Store that's not tied down to a contract, can that phone be "used/activated" on any US carrier without being "locked" to that carrier.
I'm currently with Sprint on a month to month plan. I'm choosing to purchase an iPhone outright, use it on Sprint to see if I like it, if I don't like their service, I'd like to be able to send my last check and head on over to T-Mobile with that same phone without it being black listed or locked.
No. Sprint is the only oddball that doesn't unlock their phones for domestic use. Even if you paid full price. They also don't allow other carrier devices to be used on their network, even if the hardware is compatible (ie Verizon phones).
Your only option would be to buy it from Sprint and then return it within their return policy if you don't like it.
Verizon phones will come GSM unlocked and you can use it on AT&T or T-Mobile. AT&T and T-Mobile iPhones that are unlocked will actually work on Verizon as well, but you need to insert a pre-activated Verizon LTE SIM. Verizon won't let you activate it through their system.
