T-Mobile iPhone 6 Plus APN settings

Hi could someone please post their iphone 6 plus apn setting for T-Mobile. I've searched and searched and I believe I have narrowed it down to what I'm listing below.. I'm looking for if the MMS UA Prof URL is supposed to be blank of filled in with this "http://www.apple.com/mms/uaprof.rdf"

No - it's blank at MMS UA Prof URL - no URL at all.

I have "tmus" for APN, and http://mms.msg.eng.t-mobile.com/mms/wapenc for MMSC.
