When using a knife, I am exclusively right-handed. When using a fork, it can go either right or left handed.
If I'm eating a meal which requires the use of a knife, the fork spends most of its time in my left hand. Anchor the food with the fork, cut a piece off, consume. Even for the purposes of 'scooping' food onto the fork, i still will not switch hands.
At any point the meal no longer requires the use of a knife--or there was never a need for a knife to begin with--the fork generally stays in the right hand.
Spoon stays in the right hand. Chopsticks are the same.
