If the following two questions have been covered my apologies please. I have time machine configured to my external HDD (WD My Book 1Tb) via default settings.

1) How can I back up all my hotmail folders using time machine. (Must I configure the Mail App?)

2) The top back of my iMac is hotter with the external HDD plugged in continuously. Is this normal?

I have played with the process of leaving the external HDD plugged only once per week to allow for the TM back up to occur and then ejecting. The iMac seems to stay cooler when I do it this way.
 
1. Time Machine will back up everything on your Mac, including the Mail folders (including the Hotmail mail), unless you exclude them.

2. Maybe that is due to the HDD being connected via USB, as USB is using the CPU to transfer data.
Maybe have a look at Activity Monitor.

Btw, the outer shell of the iMac is made of aluminium to better radiate the heat, thus it is okay for it to be hot.
