I just ordered an iMac for my brother through the online apple store. If he wanted, would he be able to take it to an apple store and exchange it for a better model (say 20" for 24") without a restocking fee?
 
Sweet. So if he took an unopened box ordered from the website back to the apple store he could pay the difference for a different model.

I already ordered it for him, so now if he wants a better machine he can pay for the rest.
 
You can get a full refund on new Macs within the 14 day remorse period. (here in the UK anyway)

IIRC; No, he'll have to send it back to the apple online store. They operate as different stores. If it's unopened and NOT a BTO model, then he'll get a full refund.
