I'm on a carrier unlock and I've tried the following:

I installed the profile and proceeded to configure the settings as directed above. I restarted the iPhone and I still can't send MMS. I've tried both APNs listed.
Before this I tried unlockit. ( http://www.unlockit.co.nz/unlockit/ )
There are two t-mobile options in the US listed on the site.
I tried the first, reset the phone. No cigar.
I tried the first, entered the network information detailed on the T-Mobile website, reset the phone. No cigar.
I tried the second, reset the phone. Nothing.
I tried the second, entered the network information detailed on the T-mobile website, reset the phone. Nothing.
MMS messaging is enabled within my Settings.
Please help, I am very annoyed. I have a factory unlocked iPhone 4 on iOS6 and I've searched through this forum already.
I have tried all of the above with wifi disabled and wifi enabled. In the following photo wifi is enabled, but the symptoms remain the same with wifi on or off. The message appears to be sending but fails right before it finishes.

To get started using an iPhone®, follow these steps:
Obtain an iPhone®
T-Mobile recommends you buy an unlocked and contract-free iPhone® directly from Apple®, either from their online store or at a retail location. You can also find an unlocked iPhone® on Amazon and other popular online retailers.

Select the right SIM card
Depending on which model iPhone® you have, you may need a new SIM card.
Use the online SIM checker to determine which SIM card you need.

To activate a new SIM card:
Visit http://www.t-mobile.com/SIM
Use our automated system at 1-877-234-4299.
Once your SIM card is active, just insert your SIM and turn on the device to start using T-Mobile service.
Setup internet and picture message settings
If you have access to Wi-Fi
Turn on Wi-Fi.
Connect to a Wi-Fi network.
Return to the Main Menu.
Open Safari.
Visit http://t-mobile.com/isetup.
When the profile loads, select Install.
After completing the installation, you will see the T-Mobile Mobile Life (web2go) and MyAccount applications on the Main Menu.
Use T-Mobile Internet and picture messaging settings for Apple® iPhone® to finish entering the picture message settings.
If you do not have access to Wi-Fi
Use T-Mobile Internet and picture messaging settings for Apple® iPhone® to enter all settings.

To setup T-Mobile Internet and picture messaging on your iPhone, follow these steps:
If you have access to Wi-Fi, start with Setup guide: Apple® iPhone®.
From the main menu, tap Settings.
Tap General.
Tap Network.
Note: For iOS6, tap on Cellular.
Verify the following settings:
Data Roaming: On
Wi-Fi: Off
3G Enable: Off
Note: The Apple® iPhone® is missing the WCDMA 1700/2100 band, and cannot function on the T-Mobile 3G network until the network reallocation has been completed. See: T-Mobile Steps Up “Bring your own iPhone” Efforts.
Note: Changing the 3G Enable function while the device is on an active call may cause the call to disconnect.
Tap Cellular Data Network.
Configure the internet settings under the Cellular Data section:
APN: epc.tmobile.com
Note: For some iPhone unlocks you may need to use wap.voicestream.com.
Username: <Leave Blank>
Password: <Leave Blank>
Configure the picture message settings under the MMS section:
APN: epc.tmobile.com
Username: <Leave Blank>
Password: <Leave Blank>
MMSC: http://mms.msg.eng.t-mobile.com/mms/wapenc
MMS Proxy: 216.155.165.50:8080 (Leave Blank for 4S)
MMS Max Message Size: 1048576 (Leave Blank for 4S)
MMS UA Prof URL: http://www.apple.com/mms/uaprof.rdf
Press the Home button to save the APN and exit to the main screen.
Verify MMS is enabled:
Tap Settings.
Tap Messages.
Set MMS Messaging to On.
For iOS5 and lower, Tap MMS first.
Turn the device off and back on.
Test the connection by opening the Safari web browser and sending a picture message.
