It'll definitely come, but not from Apple.

Apple announced that developers will be able to make turn-by-turn navigation apps in the 3.0 software.

My guess we will have them the day 3.0 is released. If not then within a weak. As we have some already that are practically turn by turn just without the speech etc. So I think they will be ready for launch.

Does you have to have access to an AT&T signal to use it? Or just GPS to the satellite?

it depends on the application. for example google maps uses your connection but g-maps has everything built into the app.. nearly 1 GB.
