I went to the above link today, filled in my info, and submitted the form.
I then checked my email and already had two emails from them. The first one was to confirm I had submitted the form, and the second one was to tell me the unlock request was approved.
The email said to do a restore (the restore button, not from a backup) in iTunes to unlock the phone. When the restore finished a message popped up letting me know my phone was now unlocked
Total time from submitting the form to getting the message the phone unlocked was approx. 30 min.
I think that's the quickest service I have ever got from ATT!

PS: In my case the "AT&T Account Passcode" the form asks for was "not" the password I use to login into ATT. I had to leave that field on the form blank to submit the form.
These rules seem in line with what AT&T is already doing.
