We're currently residing in Ecuador, but our Apple Accounts are US based accounts. My account has always been a US account as I was living in the US. My Ecuadorian wife's account was an Ecuadorian Apple account, but later I changed it to a US account. She's part of my family and is able to use Apple Music, storage, etc. from the family Apple One subscription. The issue is that she's not able to complete the setup for EKG because it keeps telling her it's not available in our area. I understand that it's not available in Ecuador, but I would expect that it wouldn't matter if everything about her account is US based. We're not going to be in the US anytime soon. We have a tripped planned to Canada in February 2024. Is there any way to get the EKG setup done here? I'm thinking we can probably do it in Canada if nothing else.

You're not alone, there are many complications on the watch face that are broken since the update to 10.1, apple has acknowledged and the engineers are working for the fix, there's two other thread here on apple watch with almost the same issue

This is a regional issue. The ECG feature hasn’t been approved for use in Ecuador. It’s not an iOS 10 issue.

So even though the watch was purchased in the US and has only ever been used with accounts that are US based, the actual activation is done based on location?

It appears that way, and it makes sense when you think about it. Apple isn’t authorized to offer ECG in Ecuador. You have to think that Apple is aware of that and takes steps to limit exposure for their own liability purposes.

I would contact Apple as suggested. Or try to activate when in Canada.
