Is 3GS (old bootrom) able to unlock (using 6.15.00/ultrasn0w) on iOS5? Or should I downgrade to 4.3.5 (saved SHSH) first?
 
iclarified has complete tutorials on this. They walk you through the process starting with the device you have.

which are quite out of date and do not mention iOS 5.

I don't think you're going to find a lot of people "who have already done this", as combination of iPhone 3GS / old bootrom + iPad baseband 6.15.00 + IOS 5 is a rather unusual one.

Assuming you have a functional JB'ed phone with 6.15.00 - just run ultrasn0w and see what happens. Ultrasn0w supports 6.15.00 baseband, so in theory this should work.
 
My 3GS is exactly this combination. I downloaded a custom 5.0 ipsw that preerves the baseband after I upgraded to the ipad baseband. Ill post the url link I used to upload the custom rom that preserves baseband when I get home from work.

Try sticking with 4.1 and then download the custom ipsw i downloaded from the link i posted above. I used redsn0w btw
