Hi folks, I got an iPhone 5 as a gift--- from what I can tell by the model number/code (MD655LL/A), this is a Verizon phone?

How do I go about getting this set up so I can use it with T-mobile (if it's even possible?)

As you can probably tell, I'm literally quite clueless about this stuff. Thanks in advance!

My Verizon IP5 identifies itself as MD664LL. So I doubt you have a Verizon one.

Depending on what model of the Verizon iPhone 5 you have the MD number will be different. For example:

Apple iPhone 5 16GB White Verizon MD655LL/A

Apple iPhone 5 16GB Black Verizon MD654LL/A

Apple iPhone 5 32GB White Verizon MD659LL/A

Apple iPhone 5 32GB Black Verizon MD658LL/A

Apple iPhone 5 64GB White Verizon MD665LL/A

Apple iPhone 5 64GB Black Verizon MD664LL/A
