I'm looking for a easy to use video format converter to format a ".m4v" file to a format that iTunes or Quicktime will recognize.
 
also, any video converter, free and safe, u can try

And .m4v should be readable by iTunes and QT Player (7.x.x and X).

OP, have you checked the Open With dialog in the Get Info window of one of the .m4v files and seen if it might be associated with another software?

Yes but the OP just said that s/he wanted to convert to a file that itunes or quicktime would recognize. If s/he uses Handbrake to convert it to a .m4v using one of the itunes presets it should be recognized by itunes, no? At least everyfile that I have made with Handbreak has.
 
I would suggest Roxio Crunch. I use it all the time to convert. just have to keep the right settings. you can find it many places just search lol
