I'm looking for a easy to use video format converter to format a ".m4v" file to a format that iTunes or Quicktime will recognize.

Try Handbrake. I use it all the time to convert video to .m4v for itunes and my iPhone.

What is the name of that converter again?

And to P Mentior: The OP mentioned s/he wanted to convert from .m4v and not to.

And .m4v should be readable by iTunes and QT Player (7.x.x and X).

OP, have you checked the Open With dialog in the Get Info window of one of the .m4v files and seen if it might be associated with another software?

Yes but the OP just said that s/he wanted to convert to a file that itunes or quicktime would recognize. If s/he uses Handbrake to convert it to a .m4v using one of the itunes presets it should be recognized by itunes, no? At least everyfile that I have made with Handbreak has.
