When I boot up my mac the volume settings are not preserved. The volume is always set to 50% of the max volume when I boot up.

I've run repair permissions and Onyx, but this did fix anything.

Is there a fix for this?

Is it time to reinstall Snow Leopard?
Im hoping my time machine backup doesn't remember these obscure settings or problem files.

May I suggest a PRAM reset? The article lists speaker volume as one of the factors controlled by PRAM. Reset it, adjust your volume to the desired level, then restart normally to see if it preserves the volume settings.
