I see you are saying that you have done the unpair/re-pair process, but then you seem to be referring to Bluetooth which has nothing to do with it.
Also you are saying that you are loath to start afresh, which is exactly what the unpair process will do.

Maybe I am misunderstanding, but did you actually complete the actions detained on the page below?

It is an annoyance, but it will not take much longer than half hour to complete the entire process to re-pair the watch.
The Bluetooth remark was an error of mine Which waw74 pointed out for me.
By saying I am loath to start afresh, I am referring to deleting everything on both the watch AND on the iPhone which was recommended to me.
Reference the Unpair/Repair, yes I carried out the process as in accordance with Apple’s guidelines.

In my case Bluetooth is not involved. I did try the unpair, repair trick with no luck. Apparently the problem is caused by Family Sharing on the App Store. Only the apps purchased with the account on the iPhone linked to the watch work but those "shared" do not. I can download free apps directly on the watch but I'm not willing to pay twice for the others.

Yes I did the unpair and totally wiped out watch, and deleted backups on the phone in the watch app and delted the watch app and reinstalled..Apple told me I did everything correct with all the stuff I tried.

do not have family sharing on it

So after a few weeks of apple collecting all her data from watch and phone and sending info to the engineers, they called back yesterday and told me issue was addressed in the most current watch update and to try it.... still doesn't work, doing the exact same thing... unreal

Snap... Still not fixed.
It’s annoying every time an app is updated on the phone it doesn’t get updated on the watch.
As such you have to look out very closely at what apps are being updated on the phone, then go to the watch and delete the watch App version, followed by reinstalling the watch App..... NOT IMPRESSED APPLE.

iPhone now on 14.2 and watch on 7.1
I am still unable to install apps or have apps already installed updated on the watch.

This is a nightmare having to check every App that is to be updated on the iPhone if it has App is on my watch.
If the iPhone App has a watch App the only way the watch App gets updated is for me to then uninstall on the watch and reinstall manually again on the watch.

The iPhone simply will not update any app already installed on the watch!!!

after unsuccessful tries, Apple had me send the watch to them for repair or replacement.. so we shall see what they say

I honestly wish you luck, although I can‘t see it being a hardware issue.
When I first reported the issue I was using an iPhone 11 Pro Max. Now I am using a 12 Pro Max and the issue for me is still there. As such one would be forgiven in saying “it’s a watch issue”. However, I am convinced somehow a software ‘gremlin’ has crept in and refuses to take it’s leave.

Now I am pissed.. They sent it back saying could not duplicate and it was working fine. Several weeks ago apple recorded the issue and sent it off to the engineers. I am going to try and pair it to my phone this weekend and see if it works. Maybe my daughters phone has something weird going on with it... But then again I could get apps to update from the watch itself either so I have no idea at this point.. Just sucks cuz my 13 year old bought this watch with her own money $180..
