Weird stuff going on in OSX & Brightness buttons not working,

I need help, my mac books seems to be weirding out.

Several things happening, including:
1- strange black boxes appearing around icons etc (see pics)
2- Double, "restart" "shutdown" and "log out" appearing in my menus (see pics)
3- brightness keys not working.

Please help me fix and or all of these problems, I would really appreciate it!!!

for the first two things, you might have some kind of universal access option selected.

The first two are 99% likely to be a voice setting turned on in Universal Access.

Open System Preferences and check it.
