What do apple expect when we hand iPhone back on upgrade program? Box? headphones?

Considering I've already lost the stock headphones and the box! Any idea what we need to give back in order to get our upgrade?

I don't know about Apple but I only gave AT&T my phone. No box or accessories were required.

I'm on IUP and when I upgraded to the 7 this past Saturday, I just turned in the phone. I still have the box and all unused accessories at home.
