is it just the sticker with all the barcodes with the part and serial number? or the one with your address and a different barcode?

UPC is Universal Product Code. It's the bar code thingy that appears on all sorts of consumer goods. You want the one that looks just like the bar code you would see on a can of beans, any other format is some kind of manufacturing or shipping label.
