I want to get my iphone replaced at the apple store, because of a couple of issues. Do I need to bring everything that came in the box? And do you need a receipt? Because I don't recall ever getting one in the first place....

I'd bring it in the box it came in, just to be safe. A receipt would be a good idea also. The apple store probably emailed you a copy of the receipt, so check for that.

I had neither the receipt or anything else but the phone.

They can look up the receipt with your credit card. They don't want the other stuff.

You walk out with what you came in with.

I've always brought just the iPhone, but that meant that my original iPhone box always had a different serial number on it. But that's just me...

Yes. The warranty is provided by Apple. AT&T is simply a reseller of an Apple product.
If you were in the UK you would usually go to the original retailer for a replacement, taking some proof of purchase (the original receipt is not obligatory) and I would take the box and all accessories just in case you got a new complete boxed iphone.
