When enabling WiFi calling, iOS throws up a form (see attachment).

This form is pre-populated with my old address (from 4+ years ago). If I change the address to the new one, subsequently disable WiFi calling, and then re-enable it, I get the same form once again and it is still pre-populated with the old address.

Where is this form picking up the old address from?

An address you have on file with your carrier. It is possible your new address is not saved properly.

When I first enabled T-Mobile WiFi Calling, I had a pop up as well to fill out the address but the address sticked. You can try to find the E911 address through your carrier app. On T-Mobile app, it is under my profile setting.

I checked my carrier's app and it shows my current address, not the old one.
