Where does an app developer gather my personal information?

Before downloading and installing the apps there is information suggesting that the app developer may gather my name and contact number. Where do they obtain these information? I have a different number for my inserted SIM than the one registered for Apple ID. How about my name? Is it obtained from my Apple ID?

If I change the name to a nick, will the apps still be able to get my full name? I purchased something with my Apple account with credit card before.
