I came across this picture here at Macrumors, and it shows the exact monitor mount I want for my iPhone.

Would anyone know who makes it, or it’s model name please?

Those aren't out yet to my knowledge as they were introduced at WWDC but I'm sure Belkin will be one of the first as they have exclusivity on a lot of Apple products and are sold in their stores.

Exactly, they are made by Belkin, but Apple said at WWDC that they will be available later this year (probably around the fall)

and, since the item in presentation is obviously a prototype, or some other class of pre-production, the final retail version may differ significantly.
