Thanks thought I was going crazy. I try to keep up and whenever I delete on phone I delete on watch so it does not stack up with old messsgesI have the same complaint… when I delete a message in my phone, it disappears from my Mac and iPad as well. Those three devices are always in sync, but all messages stay on Apple Watch. Very frustrating. I have yet to find a solution.
Oh, thanks! That’s helpful. I was trying to keep up with deleting as I do on iPhone as the OP mentioned, but there was no way I was going to keep up with that. This is expected behavior. The watch doesn’t use ‘Messages in iCloud’, so it doesn’t sync the same way as other devices do. Only messages from the past 30 days are kept on the watch. Anything older gets deleted automatically.

Good to know they don’t just stack up indefinitely on the watch!
Weird to read this situation, because for me it’s my 2019 Macbook Pro that doesn’t sync messages, at least half the time. It’s on the latest OS and not super old but it’s the one device where “new“ messages pop up, at least for a few minute or so, whereas they seem to sync fine between phone, watch, tablet, etc. Sorry can’t help your situation, beyond the usual ideas / tasks which can be a hassle and maybe not work, like trying to deactivate and re-pair the watch… 🫤
