One problem is the design of the housing you are supposed to grip when plugging the cable in or out. They are typically made to be as small and "invisible" as possible and typically have a slick gloss finish. If they were slightly larger and had a ridged or textured finish so you could easily get a good grip, more people would be inclined to use them rather than yanking on the cable.
 
Never had one fail on me. All of mine look brand new. I don't know what you're doing with them so I can't really help you.

All cables have a lifespan. Usually any cable eventually will fail usually because of failure of the connection between the cable and the connector, mainly because of repeated bending. Some companies will advertise what they call the "bend lifespan" of their cables, aka how many times you can bend the cable before it fails. However, eventually it will cause the failure of the cable.I believe the correct engineering term is "cyclic loading".
On the other hand, static cables may not be affected by this, in that case they will probably fail because of corrosion.

So, paying 100s of $s for a better made cable will only expand its lifespan but it would not make it indestructible. On the other hand, what is a point of an indestructible cable, when, most likely than not, it would be absolute in a few years time?

I've had many different brands break. Mostly likely due to my kids pulling from the cable.

I only use genuine Apple cables. Never in 7 years had a single one break or fail. Just treat them carefully - always unplug holding the end casing near the lightning connector, don't ever pull the thin part of the cable itself.

The connector itself is admittedly hard to grasp at times.
