The unlocked iPhone 4 had a $50 premium so I would assume they'll do the same with the iPhone 4S.

At the moment the Apple.com website doesn't mention it however the UK one does, so maybe not quite at launch

Yep. Available in Nov. I can't seem to pre-order it though. I guess this function has been disabled unless I'm missing a step.

I just called the local Apple store. Unlocked 4S will not be available until Novemeber. You cannot order one yet.
