Will not backup to iTunes when plugged in by cable

My iPhone and iPad do not automatically backup/sync to iTunes 11 when connected by cable.

I have to manually initiate the backup by pressing the Sync button in iTunes.

The iTunes preferences option is not inhibiting this.

Any ideas how to fix it?

PS: it does work automatically 20% of the time!!
