My missus reckons Coventry, especially the inner ring-road. She says it's pretty horrendous. But then she's not from round those parts...
 
brussels...and then tokyo...
brussels because belgians don't care about other drivers on the road (especially in downtown brussels)....and no. 2 tokyo because the roads are usually too full you're better off taking the subway;)

I'm gonna have to say Montreal. It's the only city in Canada where you can't make a right turn on a red light, but it's acceptable to go through it.
 
My worst three are Rome, Rome and Rome. All we had to do was drive in from the airport to the hotel near the edge. We got completely lost, scooters and cars carving everyone up, it took over two hours to find the hotel after constantly stopping to ask for directions. Never again!!!!!
