I can see the USB DAC plugged in because it shows up when I go into system report but the speakers don't show up in system preference.

The DAC is a Audio-GD NFB11.32 and there are no drivers, just plug and play.

Tried PRAM and SMC reset but no luck.

What should I try?

I know this is going to sound silly, but have you tried plugging it in to a different USB-port (if available)?

Yup.

The MBP mid-2014 only has two USB ports and I've tried both
 
Fixed it

I restored default settings under energy saver... I guess the USB ports were going to sleep or something??
