What a challenging scenario this was. I thought for sure that this attach/SKU was dead but I figured I'd invest some time to confirm because the Internet has no proof this is still a thing. Anything I found out there referenced M1/M3 MacBook Pros but nothing recently except $14.99 a month or $149.99 a year.
After over an hour talking to 3 different Apple Reps, I finally purchased it this morning for $399 which works out to $11 a month. AppleCard purchase so I got $11 back (3%) so first month was effectively free. You have to call up and ask for the Fixed Term Plan of $399. They don't know how long it is. 2 of the reps assumed it was only for 2-years which obviously is wrong because 2 annual plans would be just $299.98.
I persisted and now I have a 3-year plan on my MacBook Pro. I can't confirm this but on my M1 Max, when the 3-year expired, Apple's System Settings gave me the option to extend it for 1 year for $149.99 and it renewed again for another year so you could extend it annually after your 3-year plan expires.
