Companies like RED hire such 4K projectors, can't see why Apple would be any different. Given Apple have such a foothold on movie/video they may even have purchased one already.
Sorry, can't really see the issue being the cost, but I'll leave the implementation details to the pros.
Actually HDMI 1.4 supports 4K resolutions already, I was just wondering if we'll see that supported via the standard HDMI dongle.
Apple could easily used a customized IPad to have the required output.
Also, those cinema projectors can escalate images and use filters like blur or other algorhythms to produce a great image.
Without these enhancements, movies at digital projection theaters would look so bad and pixelated, considering the size of the screens.
Regarding the iPad 3, I'm sure in the beginning it will output HDMI and will scale down depending on the source.
Most likely 1024x768 for reglar apps unless the app can auto-escalate it's output resolution (this may be handled by iOS), and up to 1080p for HD Video.
Output may go as high as the large Apple Cinema Display, which has enough resolution.
All these could be supported by the hardware, but not enabled in iOS until a future release.
Keep in mind that apps could use an external monitor as a secondary monitor, which could have a different resolution than the iPad.
It will all depend on iOS to allow it.
