Knowing Joan Rivers and her extreme sense of humour, that's the kind of thing she would have arranged. Tell her assistant to make a few tweets that will get noticed from the other side.
 
Obviously this was not scheduled before River's death, unless you believe that Apple would reveal that there are two sizes to some random PR person weeks before the keynote.

Most likely one person is mass posting these to many, many celebrity accounts and didn't pay attention that one of the accounts should have been removed. (Also, the picture doesn't show a 2010 iPhone 4.)

Embarrassing for everybody involved, including Apple. I hate advertising, especially this sneaky kind.
 
Sorry, but I don't believe that this has anything to do with Apple. I mean would they really pay someone to call another one of their brand new products 'the fat one'. I highly doubt it. Plus, they'd want to promote the iPhone 6 with an actual photo not a photo of a 4 year old device.

I agree, it looks hacked/faked. Or at least not something Apple would arrange, and if they were paying, I'd think they'd want some control over the message.

Yes it is, apple paid apple used it even after she was dead. Either they didnt know it was automated or something else went wrong .

Not necessarily. There is an app/program/script/whatever anyone can set up that will continue to post as if it's really you, after you die. It analyzes your previous posting style and will continue autonomously.
