As you are saying slide to upgrade issue is happening again & again. So try these simple instructions.
  1. Connect your iOS device to a computer. Then make sure that iTunes is open.
  2. While your device is connected, press and hold the Sleep/Wake and Home buttons until you see the Connect to iTunes screen.
  3. When asked, choose Update.
  4. When the restore is done, finish the onscreen steps to set up your device.
Mostly it happens when your phone did not attempt to download update properly, try to update your phone via iTunes.
I hope this will resolve your issue.
Thanks but the update was done though iTunes. I tried it again, still same issue. I think the fact I have a passcode plays a role.
