If you don’t use cellular or wear a GPS-based Apple Watch, I usually know if I left my iPhone at home based on the connection status.My method wasn't my idea, but rather the result of leaving my condo without the phone on several occasions.
When I go out I'm usually walking and put my Bluetooth earbuds in and start listening to a podcast. When I get about 30 feet down the hallway, I lose the Bluetooth connection and realize that my phone is still in the condo.

I can’t leave the house without my iPhone with me, and in addition to that, I don’t set up cellular on my Apple Watch either as you have to pay an extra fee for it.
I do remember the mnemonic term for the phone keys wallet.Start saying "PKW" before leaving. Phone, keys, wallet.
Personally I 3D printed a tray with slots for my wallet, keys, phone and blade. Each item has a designated space (like a cafeteria lunch tray but smaller). Its mostly so I don't lose those things when I get home and change but it also reminds me to take everything I guess....
This is a very unpopular choice, but I use a wallet folio case for my iPhone, and so I have my driver's license and a spare credit card with it 24x7 (I also have a few more cards but I won't admit to those). I find this kind of comment, and especially the amount of positive votes it gets a bit disconcerting. The OP might be forgetful for reasons utterly out of their control. They could be naturally a bit scatterbrained, easily distracted in a way that complicates their life, consumed by worries, other problems etc. A little bit of compassion might go a long way in not making the person feel somewhat worthless or less than others. I say this as someone who is a little bit like the OP.Some presence of mind on your part.
