I put that request in too... waited and waited and waited. Finally I just called them, they asked for the IMEI and I gave it to them. One minute later they said OK, you need to do a software restore in iTunes then the phone will be unlocked. They were right, my 4s is now unlocked.

This. I filled out the online form, waited 7 days, and nothing. I called in, and took me 3 mins to reach someone who unlocked in real-time and asked me to wait an hour to restore. I restored within 30 mins and I had an unlocked iP4. I'd suggest you call in instead of filling out the online form.
 
I am under contract but bought an iPhone 5 full retail would I be allowed to get that unlocked? I meet all the criteria just wondering about still being in a contract. Thanks.

And another question...once it is unlocked, how will I know that it is in fact unlocked? Thanks guys.

Since, you purchased the iPhone 5 at full retail. AT&T will be able to unlock it for you. However, if you purchased a "device only" and don't give them AT&T number from the Apple Store the phone is already unlocked.

Once, you restore the phone in iTunes you will get a message stating "Congratulations your iPhone is unlocked" message.
 
It took AT&T nearly three weeks but I finally got my unlocked email today. It tells me to wait 24 hrs after the email to restore.

My iPhone is contract free on AT&T and has been since May and I'm not sure what to do with it now.
