For the last 2 months, every call I make to my girlfriend's phone (iPhone 6) goes to voice mail for the first time and then it rings on the 2nd try. Do Not Disturb mode is disabled.

Also this is happening to my boss's phone (not sure the model, but it's an iPhone) but they are both using T-Mobile.

Does this seem to be a carrier or device issue?
 
Do you know if WiFi calling is being used?
Good idea.. I'll check this out once she comes back from work.

I would say TMobile. Maybe they can reset their voicemails.
I don't think it's an issue with the voicemail per se.. it feels like the iPhone is acting like if it's on Do Not Disturb mode where it only rings on the 2nd ring. It still could be a problem with T-Mobile, but I don't think it's their voicemail system.
 
So... I'm really embarrassed. Upon checking my girlfriend's phone, I found out DND mode was enabled! She wasn't even aware of such setting, so it was probably enabled by accident. I thought I had checked that before but... oh well.

Happened to me once too. Make sure it's not scheduled to start at the same time the next day!
