For me the default seemed about right. I like that I can tap it when my wrist is down on a desk or even move my wrist around when driving. A good thing is I noticed I have it at the display angle at other times and it does not stay on. Could you imagine if you had the wrong angle in a nap and it stayed on?
 
There definitely needs a setting adding to adjust this. It's either a bit oversight or they left it out on purpose to prolong battery life.
 
So I think we found a workaround for now. He set up a stopwatch complication And starts it before his shift. When he raises his wrist he places a finger on the watch face while he uses the the hand with the watch on to get the patients pulse. As long as your finger is on the display it will stay awake. Obviously not ideal but a decent way for nurses to still use this.

Yeah this won't work for me. I use my right hand to hold a doppler under a big pregnant belly and my watch hand to count heart tones. Good thing I have two students. I'll make them count.

I know this is a really old thread, but Watch OS 2 allows you to select 70 seconds for the On Tap Wake screen.

Go under Settings > General > Wake Screen in the On Tap section, choose Wake for 70 seconds.

From reading the description, it says it will stay on for 70 seconds when you tap the screen. (not when you raise your arm, which goes off when you lower your arm, as it did previously)
