I chose T-mobile as my carrier and then was prompted for my account information to verify my account with T-mobile. Unless I'm mistaken, you have to do that if you select one of the carriers - the only way to avoid that is SIM free and activate with a carrier later. The offer terms on Apple's website has the following question:

"How do I get the offer?
When you buy your new iPhone from Apple, choose T‑Mobile or Sprint as your carrier, trade in an iPhone 8 or newer, then activate with any T-Mobile/Sprint phone plan. Once your order is complete, you’ll get an email with next steps."

My reading of that is that if you didn't select T-mobile or Sprint when purchasing through Apple (and trade in an iPhone 8 or newer), you won't be eligible for the offer. I could be wrong, but I don't think I'd want to chance it by buying the SIM free model.

Edit: I also just saw this question on the same offer page:

"Which phone plans are eligible?
You can activate on any T-Mobile/Sprint phone plan. To be eligible for the offer, you will need to authorize activation when you buy your new iPhone from Apple."

I think that means you're out of luck if you don't authorize activation at the time of purchase.

The promotion code mentioned above now works when I type it in after logging into the promotions site. When I have the phone in the AM (which I have to get the store person to do the trade in on because I didn’t initially do that when I ordered because w/o the extra $150 it wasn’t really worth it) then I’ll submit.

I submitted mine this morning now that I have the phone. The trade-in was on a separate receipt because they couldn't unbundle the various components of the pre-order, the activation, the trade-in, the pickup/reservation schedule at the store and so I'm sure I will have to explain this to whomever reviews the submission at T-Mobile. On the upside, the phone is beautiful and is very easy to hold (12 Pro/Blue). Interesting note, the submission didn't ask me to upload any receipts.

How did this work out with your new phone purchase being on one receipt and your trade-in being on a seperate reciept? Did T-mobile give you the rebate? Was there any hassle? I am in the same position if I pick-up my phone tomorrow.

I believe it's 2 weeks after apple receives and approves your trade-in
 
I'll just update my experience. I never received an e-mail from Apple or T-mobile with the promo code, but I used the one earlier in this thread (I chatted with Apple Support to confirm the promo code as well). I waited to submit my T-mobile promo code until Apple received my trade-in and approved the trade-in value - that was Wednesday, 11/4. I got the electronic prepaid Mastercard 3 days later - Saturday, 11/7. Very quick processing and approval of the rebate for me. Hope it is for others too.

Anyone have success with the $150 rebate buying a t-mobile version with “activate later” option? My account type wasn’t able to activate at the time of purchase.

Also, does anyone know how long you actually have to keep the T-Mobile sim in the new phone to qualify? Everything I’ve read just says it has to be activated on T-Mobile without mentioning any amount of time the line has to be associated with the new device.
 
I am in the same position as you. I ordered SIM-Free before the promo was published. Apple Chat rep said I could cancel my order but it would delay by a few weeks, and then I got ghosted by them on chat after giving me the generic link to the promotions.t-mobile.com site but not providing the promo code itself. I got put back in the queue and am now 1 hour into waiting again. Can you please share the promo code either here or via DM? Thanks.

$550 for a phone trade in (11 or newer I think... $415 for the tier below)... plus another $200 if you’ve been with TMobile for 5 years+. Even without the 5 years the trade in offer is still better at TMobile.
 
Still waiting here too. Just says "Received", but my Apple trade-in was completed 13 days ago.. Maybe another week?
I just chatted with a T-mobile rep and was told that it will take between 4-8 weeks for the gift card to show up.

Apple gave me $450 for the trade-in plus a $150 virtual gift card from T-mobile.

As I had to buy mine with Activate Later from Apple, both lines seem to be stuck as Received in the T-Mobile promotions system, so the account rep on the phone credited the $150 to the line. It took a number of calls, but finally got all the lines credited appropriately this way. I never received any of the actual virtual gift cards, but it came out right in the end. YMMV.

Just an automated text when I registered for the promo at promotions.t-mobile.com and then another saying the gift card was ready.

I purchased an Iphone 12 directly from TMobile. I had an Iphone 6s Plus. My wife and I historically were AT&T customers. However, the TMobile deal was too good to pass up. We signed up for the over 55 unlimited data deal. Even by only purchasing one phone (my wife is keeping her Iphone SE), the trade in of my old Iphone resulted in a $320 credit amortized over 30 months. So, with a $10 monthly discount when signing up for Autopay, the monthly price of service for both phones is $90. This gives us ulimited data in addition to voice and text messages. With the monthly credit discount, the cost of my Iphone 12 with 64g memory is $15 per month or, if you are paying attention, that means I am paying $450 for the new phone. As the AT&T commercial says, "I can work with that."

TMobile is being very aggressive in their pricing......obviously trying to buy market share so that they can sell more services to their customers in the future
 
What a **** show.
I have a third device that the trade-in has been fully received by Apple and completed, and the T-Mobile promotion still has not been updated beyond "Received" so I am going to have to call in again and get what will probably be a $150 account credit. This is the last of these for me, thank goodness. On the upside, once I explain what's been going on, I get the credits applied relatively quickly.
