Got my refund email today, and my ship confirmation for my free case today. Life is good.
Is anyone else unable to get past the "Get Started" button? I've been getting "Unable to process your request. Please try again later." For hours. I've tried deleting and re-installing, no dice.
Any ideas?

Probably not. The program ends September 30th, and you have to download the application onto your iPhone and request one via the app before the 30th, but you won't have your phone by then.

I am on the same boat as you, when I get my iPhone I will still download the app if it's available, if not I'll just call them with my receipt in hand. Either way, we are eligible so technically they still have to give us one.
No they don't... if they did, I wouldn't be asking here.

My free case broke a week ago. The tabs holding the two pieces together just gave out.
Consequently, I've been having serious proximity sensor issues and went back to get my iPhone 4 replaced under warranty.
For gits and shiggles, I reinstalled the iPhone 4 Case app on the replacement iPhone and was able to order another one, probably because it detected a different serial number.
It seems that it would be a little far-fetched (and unethical) if you wanted to get a different one, but at least it's an "option."
After the September 30th, the Case Program app will still be available to download but you still need to order a case within 30 days from the purchase date. Now, I have no idea when your purchase date is, either the time you ordered it or the point it ships and charges you (if you ordered at apple.com).
Since you ordered it on September 13th and you will get it in 28 days, you'll have two more days to order a case (this is what im thinking)
OR just simply call Apple, its easier to call than to ask here about that.
Since my phone won't come in until mid-Oct I will try the app, if it doesn't work I'll phone them up.

the app will probably be gone by then, i think, by what says on apple.com

Now this is what I call weird. I sold my 1st iPhone and bought another one about 2 weeks later from a seller on CL. Everytime I tried to use the app again I got shut down. I have enough cases just wondered why it never worked for me. Guess it wasn't meant to be.
I will indeed since technically I'll still be within that timeframe since I purchased on Sept 19th.
I purchased my iPhone on Oct. 6th. Just installed case program app (link here) and it let me select a case and order. If you search the app store for case program app, you will not find it.
There were no Apple bumpers available to choose from so I got the Griffin reveal etch graphite based on a friend's rec. Shipping says 1-2 weeks.

I'm sure they'll have it up until they deplete the 3rd party cases inventory they must have had for the free case program.
I ordered my iPhone 4 from AT&T on September 28 and ordered on October 2nd. My Incase Snap in smoke/black frost came in October 8th.
