This looks like Tiger. Looks like, OP, what OS version is this? Also, what did you do just before it happened?

markyr17 macrumors 65816 Aug 29, 2014 #6 I've had it happen to me a few times myself... It's just a glitch in the OS. Actually, though, I can't remember it happening in the Yosemite DP, so it could be fixed with forthcoming updates
