It’s not a notification that the app is running. It’s a notification that you are available. When you are available, if some one “calls” you, you will hear the message immediately. This can be annoying. Toggle it to “unavailable”, the notification will go away and if anyone contacts you, you will first get a message asking you to be available before playing the message.

This is especially important because walkie-talkie will IGNORE do not disturb. So I see Apple's reasoning here based on the decisions they made. But I also highly disagree with those decisions. They need to change it so that your availability times out after a certain period of inactivity. Or better yet, make that a setting with time out values of 15 minutes, 30 minutes, 60 minutes, and never.

It's gone in watchOS 5.1.2, which is in beta now. It appears briefly then disappears.
 
That's because in 5.1.2 beta that have added a ON/OFF Walkie Talkie toggle button in the slide-up control panel! It should be awesome!
I agree. I rarely use walkie talkie, so being able to turn it on and off quickly from the control center is a much needed feature. It will make it easier to enable quickly when I want to use it.

This is the type of thing that will finally get me to use this feature. Launching the app to change your availability is way too fiddly if you don't have walkie-talkie setup as a complication.
