It is likely that it did not pass initial Quality Control the first time. Apple sets this phones to the side and makes necessary repairs to pass inspection. Looks like you got one of these phones. I would NEVER accept one of these phones if I was within my 14 days exchange period.
 
Actually Apple does not refurbish devices. They give out brand new devices labeled refurbish. I worked as an apple genius for awhile. Never got an answer why but that's how it is.

I had an issue with my plus 12 days after I got my phone and Apple replaced it with one out of a little white box. You think it's new?. I'm asking you since you said you worked as a genius. Thanks

Yea it's new. They don't give out refurbish phones for fear of getting back another damaged device and worse an upset customer.

What happens to the many thousands of phones that inevitably returned after being opened and exchanged or returned for various reasons? It seems like it'd be a huge waste to destroy them when most are fine and have expensive components in excellent condition that could be re-used.

Hell, years ago I briefly made a business out of re-purposing the useful components out of non-functional desktops and laptops and just fixed what needed to be replaced.

I doubt it. Maybe when the phone is still new. But if you work as a tech you know what a refurbish device means. That means updated like new used device.
OP is the box completely different from the new box? If you were under the 14 day no questions asked, I would not settle for anything less than a new replacement.
 
Chill dude.

I just had my iPhone 6 replaced (dead pixels) for the same white box and it has the same C39 serial number as my last device. Refurbs are assigned a different starting number and its too soon for refurbs, damn they can't even replace screens yet.

Also, my white box replacement is two weeks younger than my original device.

He got sold a phone with dead pixels, don't act like apple is some kind of saintly company for replacing his defective phone.
