Right pocket and facing away. The power button's placement depends on whether I've got headphones. If not, the power button is down. When I pull it out, it's ready to be used.
 
Right pocket, screen in, upside down (unless using headphones).

Screen facing in to avoid any damage from bumping into things. Wallet goes in the left, if I'm carrying keys they tend to go in with the phone (keys on the outside, so the phone is protected by the case on its back). Upside down so it's ready to use as soon as I take it out.
 
I'm EXTREMELY picky about this. I've had the exact same habit with every phone I've ever had and I can't think of a single time in the past year that I've ever done anything different.

iPhone goes in the front left pocket screen facing in and dock connector up. Keys front right pocket and super slim wallet in the back right.

I never keep change on me and anytime I get coins back from a purchase, I QUICKLY dispose of them in the nearest tip jar or donation bucket I can find. Spreads the love and keeps my iPhone happy and free of scratches.

Only horror story I can think of is when I had an iTouch in my right front pocket and my LG phone in the left pocket in the usual fashion. (Before my iPhone days). I slipped my work badge that has a metal clip into the same pocket as the iTouch and when I used it next, there were HUGE scuffs on the screen. I couldn't believe my badge did that.
