I travel about 18 trips a year... the only time that I use my MBP. Most of the time I am using hotel internet or airport wifi, so lack of tethering is not a problem.

Every once in a while, I need tethering, say 8-10 times a year that I would use it. I would pay $60/year extra to not have to worry about connectivity. I think that there are many people like me who would automatically pay a small amount based upon the potential of needing it.

There is no way that I would pay $20-$30 per month for something I would rarely use.

If AT&T makes it cheap, I think that they will have a large adoption rate and people like me will essentially be paying for the more power users of bandwidth. Everybody wins.

My lifetime use of bandwidth for original iPhone, 3g, and 3gs is 148 MB of uploads and 2.2 GB of downloads... hardly maxing out the AT&T servers.
